The Throne of Destruction is the final level in the Worldstone Keep. The previous three levels are listed to the right in the infobox for reference.

As the final level before the last boss it contains perhaps the most dangerous combination of monsters in the game (barring Chaos Tristram). Monsters such as Oblivion Knights, Black Souls and Undead Soul Killers, all dangerous on their own, here appear together.

Baal is located at the very end of the level, however he does not fight the player here. Instead, he will summon five waves of monster, each led by a super unique monster. These are some of the more nasty super uniques in the game, especially Lister the Tormentor, the super unique of the fifth wave and undoubtedly the most challenging super unique in the game (excluding bosses and Pandemonium Event monsters). During these fights Baal will continually cast Decrepify on the player.

In order to advance to the final level, the waves must be defeated. Or rather, the waves must be removed from Baal's presence. That can either be done by killing them or luring them away from him. If at any point there are no monsters near Baal, he will immediately summon a new wave. If the fifth wave has already been summoned, he will open the portal to The Worldstone Chamber. Thus, if a player is unable to defeat Lister, Baal can be reached anyway. This can be helpful, as depending on class, build and equipment Lister can actually be harder to beat than Baal himself.
