The Worldstone Keep is the last dungeon of the game, protected by three Ancients, where Baal is seeking the Worldstone. The entrance is sealed until those Ancients are slain. The fourth level, the Throne of Destruction, is also a part of the dungeon, but is listed in a separate article. The size and level of it is included for reference in the infobox to the right.

Perhaps because it is the last dungeon of the game, a host of different monsters, some of the most dangerous in the game, spawn on the various levels here in Nightmare and Hell. Death Lords, Burning Souls, Serpent Magus and Oblivion Knights all appear here on different levels.
